Reimbursement and Patient Assistance Program

Abbott is pleased to introduce the Zemplar Resource Center, a reimbursement and patient assistance resource that ensures all who can benefit from Zemplar are able to access treatment. Available to physicians, staff members, patients, and caregivers, it's your source for understanding Zemplar insurance benefits and finding alternate funding when needed. The Zemplar Resource Center can help with 1) facilitating reimbursement, 2) identifying alternate funding and 3) obtaining Abbott sponsored medication assistance.

1. Reimbursement Services

Our reimbursement counselors will help patients understand and access their insurance benefits by:

• Minimizing unnecessary delays for patients to receive Zemplar treatment once it's prescribed.
• Contacting insurers to determine insurance coverage and availability of benefits, as well as coverage policies specifically for Zemplar.
• Providing information and general assistance regarding insurers' pre-certification and prior authorization procedures for Zemplar.
• Facilitating and providing information about insurers' appeal processes for coverage denials.

2. Alternate Funding Searches

We realize not all patients have third-party insurance or benefits that cover Zemplar. Abbott is committed to making sure that we help patients explore a multitude of avenues for funding. For patients without coverage, our reimbursement counselors assist by:

• Exploring alternative sources of funding for Zemplar, such as community resources, charitable organizations, foundations, and medication assistance programs.
• Helping to determine if patients benefit from and qualify for the alternate funding sources.
• Connecting patients with the sources for assistance.

3. Abbott Sponsored Zemplar Patient Assistance

In the absence of an alternate source of funding, our reimbursement counselors will provide information and review eligibility for the Zemplar Patient Assistance Program.

• The Zemplar Patient Assistance Program, sponsored by Abbott, provides medication at no cost to eligible patients who do not have prescription drug coverage and who meet specific financial criteria.
• Eligibility is reviewed on a case-by-case basis.
• Upon approval into the Zemplar Patient Assistance Program, a three-month supply of free medication is shipped to the physician for dispensing to the patient or shipped directly to the patient's home.
• Medication can be re-ordered by the patient or physician within the patient's predefined eligibility period. After the eligibility period, patients are asked to re-apply for further assistance.
